Word 2002: Best way to use TOC with captions

Hi,
I have a table of contents but am having trouble keeping my tab settings when I update the TOC. I am using captions and then inserting them at the right margin. I've used a dot leader for the tab. Every time I update the field, which is the TOC, the tab settings are lost. Any suggestions on a better way? My TOC is not referencing page numbers, only certain areas of the document.
 
I've tried creating it with table entry's and with styles. Nothing works.
 
Isn't there other ways to create your TOC other than with styles? I've had problems using styles in this particular document and found it would work better if I used captions, or possibly bookmarks? I can't find much documentation on how to create TOC's other than using styles. Any help is much appreciated!
