Windows Protection error

I fdisk, formatted and reloaded Win98 onto the HDD. After shutting down & restarting...Windows won't load because a windows protection error occured while initializing device PTVCD. How can this be fixed?
 
Boot with the startup floppy and get to the A:\> prompt. Once there
type fdisk /mbr. Do this several times to insure that it did its thing.
This command will clean out the mbr fully.
Now type format c:/s .
Next ,
Fdisk and format , your hard drive . Do not reboot your system but
run fdisk /mbr .
Then reboot your system .

PTVCD is not a standard Windows component and there is no reference to
any such file or component in the Microsoft Knowledge Base.

However a Google web search shows a number of references to PTVCD.VXD
in connection with certain modems. So that would indicate that you
should check with your modem manufacturer for updated drivers or other
information regarding this problem.
