Voicemails show up without the extension ringing

I have a client that is sitting at their desk and suddenly their voicemail light turns on. She checks the message and it is from 5 minutes ago. The phone never rang and it was a direct call to her dedicated line from the outside. Other phones are having the same issue that voicemails are coming in without the phones ringing. It is very intermittent. The IPOffice is a 2.1 with voicemail pro. Thank you.
