USB Devices Restart the PC

This is an ongoing issue and we continue to have issues with some of our PCs and the USB connections
When we connect the USB device of any type the PC will blue screen and reboot with an IRPStackSize error
I have used Several fixes with no luck below I have listed the fixes I used to correct some of the PCs
Ghost software:

and installed SP2
None of these fixes worked on the PCs consistently and I am in desperate need of a fix! any help would be great
