Upload fails

We are running NT Server 4, service pack 6, IIS 4, using an https protocal in a domain of about 500 users. On our website we have an upload feature that was working fine until about a month ago. It works successfully 100% of the time for admins. Permissions are not an issue, as they have been scrutinised and re-applied faithfully. It works intermittently for anyone else with an account on the server. The error we are seeing is error 500, internal server error, seems kind of nebulous. We can also copy a working account (we thought as a temporary work around, to use a template), but after about 30 that copied account fails about 75% of the time. Help?!!
