I am updating our master client database with new client address information from our UPS shipments on a regular basis. Rather than enter the updates manually I want to merge the info. Duplicating the field structure in both databases is no problem. I want the query to only add a UPS entry to the master database if the name is not present in master (last, first & middle initial fields).

At this point I am not trying to update street, city, etc. data that may have changed, only unique names.

Place both tables in the top pane of your append query. Join the fields you mentioned and then individually double-click on them and select to "return all records from...UPS table ..."
Then drag the UPS table fields to the grid with the Last Name field from the other table. Set the criteria under this column to
Is Null
This should append only records from UPS where the same last, first and mi are not in the other table.
