Unable to delete custom entity entry

Hello,

I'm just dabbling in creating custom entities and I am having problems. I have created the custom entity. I have created a parental relationship between account and the custom entity. Now when I go into an account and go to create an entry in the custom entity, two entries pop up and I can't delete either.

However, when I go out of the custom entity into the main screen and access the custom entity, I see the entry and am able to delete it.

What am I doing wrong??
