Trapping Spot Color over gray image

I have a file in Quark 5 that has a grayscale photo in the background with a text box of PMS 201 type over top. Quark wants to add spread to the text box. The problem is only half of the type overlaps the dark areas of the photo. the rest of the type is over a white area. Quark wants to add spread to the entire text box, making the type outside of the dark area fat and ugly. I would like to know if it is possible to choke the black in the photo instead of spreading the type. Or is selecting individual words and letters over the black and applying trap the only option?
 
I don't think you can control how the picture itself traps, but you should definitely be able to select the characters in the text box that you want to trap. You don't have to select everything.
 
That is what I ended up doing to get the job done. I was just curious if there were any alternatives. At times selecting line by line and individual letters around an image can be tedious. I didn't think I could control the underlying image but I thought it was worth asking.
