Table of Contents - Little Boxes Instead of Headings

I am new at creating Table of Contents. I have applied style sheets to all of the headings that I want to appear in the TOC, then created a list. But within the list LOTS of the headings are not showing up and in at least 70% of the places where text should be showing up, there are a series of little boxes instead.

Does anyone know what's happening? I need serious help!

Could be the font.
I know if I have a dodgy font sometimes it will show up as boxes
