I am trying to develop a survey using forms and am not sure how to build it. I have 16 questions, the first 12 are rating on a scale of 1-5, the next two are yes/no and the last two are text. I'm not sure if I should set all 16 of the questions up in a table. The part I'm mostly confused on is how to set up the first set that are based on a scale. I'm sure there is a simple way to do this. Any help or reference to an example would be greatly appreciated!

Susan, you might want to set up your 1-5 field as a Lookup field in the table. Then, on the form you could use a list box or option group to choose 1, 2, 3, 4, or 5.
