You would actually add that to your form tag. You will need access to cgi-bin to add a script at your isp (unless you are the host) as well. Unless they have pre-installed cgi-bin scripts or unless you are the host I don't see how it could be done.
There are several ways you can do this. If you put mailtoyour address), it will try and use their default mail client to submit it. Plus if you do it this way they have direct viewing of your email address. The best way would be to create a script that would do this for you. Plus you can format the data that is sent to you better.
