store.exe 100% due to a single instance of thread

I know high cpu has been discussed but specifically... what can I do when Performance Monitor shows a single thread of the information store that appears to be continuously running near 100 percent CPU usage in this case instance 10. KB article 181410 details it perfectly but we're on sp4 and the fix stated replaces two files with sp3 versions.
Anyone help at all, would be much appreciated as this has been going on for quite a while now !
