SQLCtr60 Errors in NT Event Log - Please Help

I have a server called "Invest_LM_DEV" running SQL Server 6.5 and it is constantly sending "SQLCtr60 Errors" to the event log. I have checked the knowledge base and checked everything on article "Q227662", however it is still giving SQLCtr60 Errors.
These are the errors that appear in the Log

1. OpenSQLPerformanceData : Cannot Connect to SQL Server - INVEST_LM_DEV
2. DB-LIBRARY error - Login incorrect.
3. DB-LIBRARY - SQL Server message: Login failed- User: Reason: Not defined as a valid user of a trusted SQL Server connection.

I would be grateful if anyone can help me and tell me why this is happening and how to fix them.
