Sourcesafe Change is Lost

Greetings,

We are developing applications in Access-2000 on Win-2000 workstations and using sourcesafe to manage the Access database. On several occasions we have noticed (stumbled across?) a change that has been "lost", and obviously are deeply concerned. Most recently:
I modified a module on 3/30/04 and checked back into SS.
Administrator pulled a version on 4/12/04 and sent to users (my change was present).
Two days later, Administrator pulled another version, but this time my change was missing. This problem was not discovered until today (4/29).

Looking at Sourcesafe "version History" for that module shows my change on 3/30, then another user checked in that module on 4/20. No activity logged between 4/12 and 4/14 to indicate any problems.

We just ran a test to see if there was any way to recreate the loss, but were unable to do so. (1. Only one user allowed to check out a module at a time; 2. If user makes change without checking out, they can not check-in; 3. If user makes change, then checks out, SS overlays with most current version.)

Does anyone have any screaming idea where else to look (will settle for a wisper also).
