I have a customer with IPO 8.1.43. Using Softphone. When the Softphone is logged in, a 6 digit SIP extension is auto-created. When the Softphone user logs out, the 6-digit SIP Extension that is created is deleted. The problem is that one of the SIP extensions won't delete and I cannot delete it as it's greyed out. It creates a problem when a particular user tries to log in and use the Softphone application. It will attempt to login for a short while then go into the following error:
Account failed to enable.
Account: IP Office could not be enabled.
Problem at server (SIP error 408). Try again

Any thoughts.....the users are offsite and using a VPN connection back to the main site. One other piece of info, under the Error List it shows two of the auto-created SIP extensions and indicates "An Extension with this IP address is already configured".
