My Icons on the taskbar, Windows Explorer, on the Programs section of the Start Menu and View File Types have shifted (small icon mode) to display the last portion of the current icon (50%-75%) coupled with the beginning of another icon. When I click the icon in the View File Type dialog box, the icon reverts back to it's original state (only in dialog box) but does not save that way once I exit the dialog box. Any ideas?

Search for and delete ShellIconCache which will be in the Windows folder then restart the pc. When you restart a new ShellIconCache file will be recreated. See if the icons are proper then. Other than that i'm not sure what to tell ya.

You can also try downloading Tweak UI for your Operating System. It is a free download tool that allows you to change many things on the pc. One of the options available is to rebuild the Icons, I'm pretty certain that will fix your problem. (You can search for tweak ui in google.com, just put that and your OS)
