Setting up a Form as a Template

I want to set up several templates (Forms). These templates will have data in most fields but it will be standard data. When these templates have completed data entered by users I want them saved as a seperate record...not to overwrite the template (Form). How can I mechanize this.

Forms don't "have data". Tables have data records. Forms have controls that can be bound to fields in tables.

What is it you really want and why?
