What is a good general rule to follow for servers, for example is it good practice to have the SQL Server and the Web Server on the same machine, or say, Domain Controller and Exchange Server, or is it Better to keep them all on seperate systems?

Thanks in advance, this is driving me crazy since I want to have a good expandable infastructure, I have to redesign how I have things now since I got hacked pretty badly recently and am going to deploy the ISA server and am trying to reconfigure how I have the other machines, I currently have the (web server + SQL), (Domain + Exchange + DNS), and another secondary DNS server.
