A user (Steven) has left the company and so his account has been disabled in Active Directory, his name removed from Exchange lists and any delegates set on his account removed. But for an unknown reason Steven is being secretly invited to meetings when meeting requests are sent. When other users send meeting requests to other users they will receive:
Your message did not reach some or all of the intended recipients.
Subject: Updated: PP50 Design review follow up
Sent: 01/11/04 16:53
The following recipient(s) could not be reached:
Steven on 01/11/04 16:53
You do not have permission to send to this recipient. For assistance, contact your system administrator.

This doesn't make any sense as Steven isn't an invitee, resource or any user entered entity. What could be causing this, I can't find anything within Active Directory?
