Save Conflict when saving a document with an attachment

I have a couple of databases where saving new documents is fine until you add an attachment, then when you try to save the document, it creates a save conflict!<br><br>We are using Notesv4.5.2b<br><br>Any ideas will be gratefully received.<br><br>Alice.
