Sample Patient Database 2

Hi there, I would like to know if anyone knows where I can get a sample of a Patient tracking database (track appoinments, visit information, follow up, etc) I am just trying to generate some ideas of how to set up one I need to do. I don't need to have access to any code, but I would be willing to purchase one if it was close enough to what I need and then I would like the code.

I have designed databases using Access 97 which track patient information as well as visit information for hospitals. What are you looking for. If I can help somehow I will see what I can do.

Thanks TM & Doug. TM, I have already designed the table structure, so basically I am just looking for ideas for the look and flwo of the user interface. I am not looking to use someone elses database I just want to get ideas since I am not creative in that area. Basically the main tables are in 3 "levels", it will have patient information, where one patient can have many "cases", and case information where one case can have many appointments and then information for each appointment. It is not tracking scheduling, only the patient medical information and follow up. I would love to see yours if you think it is something that would help me with user interface and appearence ideas. If your just looking for Ideas for a user interface, how it looks and interfaces, I may be able to help. Let me know how to contact you, and also let me check with the powers that be just to make sure, cause we deal with alot of security and confidentiality. but if all your looking for is appearance and interface Ill see what I can do for ya. Just to let you know the one Im thinking of was designed for the pediatrics dept. and deals with patient and visit records, along with other things which it sounds like you really are not dealing with.

Hi TM that would be great. Feel free to just delete all the data in my copy or what I do for expediency sake is do a find/replace to jumble up the key information.(replace all a's with e's) I know that sounds funny but it certainly keeps me from sharing confidential information. My email is dawnd3@yahoo.com. The database I am doing is for physical therapy patients. So the feilds may be different, but it is tracking visits as yours is. Thanks for all your help. Dawn
