Sample Appointment Scheduler DB

I’m looking for a Sample database if possible, a an “Appointment Scheduler”. I know there are some commercial products available, however we can’t use a third party commercial software product and without Microsoft Outlook. I was going to build one, however I thought as an alternate avenue to seek and resource, to see if there was already one made, it would save me a lot of work. Setting: I have a staff of 5 Members; they can see Clients Monday through Friday, 7 am to 4 pm at 30 min or 60 min intervals; some appointments can be made up to 6 months in advance. A screen would open up, viewing all staff members and their appointment status (available or filled) where an entry could be made for appointments. I would need a demographic form with follow-up summary reports. One of the reports would need to have all 5 staff members shown in a summary report of what the schedule for that day is. Also a report for all the no-shows is needed. Any suggestions would be greatly appreciated. Thanks……
