Rockwell Modem Fails at end of sending FAX

My Rockwell chip modem fails at the very end of sending a FAX to some real FAX machines. It gets to 100%, seems to send tones to indicatee the end of the FAX, and then fails. As a result, the receiving machine gets nothing. What could be wrong?
