Report from form data

This gotta be the simplest question!. I apologize ahead of time.
I have a form with text boxes, check boxes, option buttons, toggle buttons, and command buttons. I am using this form to collect data from customers. This form is not based on a table or query. I would to generate a report from the form. I would like to have a single command button to achieve that.

Example:
after all the questions are answered, a report may look like this:
This customer is married, he/she has no children. Her last visit ro our store was August 11, 2003.......

Since you do not base this on anything, how are your storing (saving) the data?? If you do not want to save the information, maybe you could set up a template in a word processor to print out and not save.
