Ok, here's the situation.

Internal usernames for our organization are:
Firsname+LastInitial

E-Mail addresses are 3LetterInitials@firmName.com

cool beans right? well, kinda.

When the person who first setup our server -- the firm hired a consultant who set it up who then hired me to work for them while I finish college --- did the "next next next" number through the create new user wizard then added the initials@firmname.com as an e-mail address. The result, e-mail sent is MarkL@firmname.com, even though MJL@firmname.com comes in just fine.

Well I realized this and when I created new accounts I create the account as normal but when prompted for the exchange alias/mailbox name I use the intials so we just get the intials which we like much better.

You'll see there is another post about changing the domain name and firm name and part of this whole thing is we want everybody using initials@firmname to send and receive. Well I have about 18 users who existed at the time the server was originally setup who have the above problem of sending with one thing even though they should be using another; new users I've created while they have other aliases for incoming all send with the initials which is fine.

As a work around for the 18 users I have set the e-mail address to be initials@firmname as the primary and unchecked the box "update based on receipient policy" or whatever it says which I can't remember off the top of my head. So I have two questions:
1. What are the problems with not updating the recipient policy for these 18 users?
2. For these 18 users, is there any way I can rename tehir exchange mailbox so that it is their initials and the whole problem is then avoided?

you mentioned the alias in your message, and this is exatcly where you want to look. When the Receipient Update policy (RUS) is run, it looks at the exchange alias to auto-create email addresses. if you want your email address to be something other than your username you specify it here. this can be found inside of AD, on the selected user go to the exchange general tab.

If you want to "rename" the mailbox, you will need to check the mailNickname setting inside of ASDI. You will want to make sure this is set to the intials you want for the email address as well.

If you need specifics on how to use ADSI, let me know and i can post some instructions.
