Remote SIP phone issue

I'm getting an issue I've not seen before. I am sure it is something I am doing wrong. When I turn NAT OFF on the phone, the phone registers to IPO no problem and I can make calls but there is no audio. In the trace the internal IP addresses are being used so that explains why the audio is not working. If I turn on NAT on the phone, with all other settings the same, registration name, password, etc I'm getting 401 Unauthorized. But I do see the proper IP addresses getting passed. I do not understand why this would be the case. This is on a 500v2 9.1.7.

What's more strange to me is that the SIP line works fine just not the remote SIP phone. Thanks in advance.
 
Hard to say without seeing the monitor logs but a SBC is highly recommended connecting any remote SIP endpoint.
