Remote Desktop / Terminal Services does not Log Off

We use a lot of Remote Desktop or Terminal Services sessions to connect to our various servers, and without exception, no matter how cleanly we go through the log off process, the session is not fully logged off. It doesn't take long for the connections to exceed the number allowed for each server -- and so one of us has to find a server to log on to from which we can run Terminal Services manager and force log-off's in there.

Any ideas?
