i have found countless issues that have caused echo. anywhere from cabling issues to bandwith issues to the handset. I figured it would be a useful thread to list all the causes of echo that you guys have found.

QoS should be implemented in all eqiuptment fraom the IP phone to the PBX - all switches etc. Lack of QoS - is a missing link - and will give "non-crystal clear voice".
