"Blip" after some cross dissolves

I'm currently working on my first project in Premiere 6 (I've been using Premiere since version 4). After some of the cross dissolves the clip that I transitioned to flashes (one time). I've already checked to make sure the transistion is going the right direction (A-B, B-A) and that there are no hidden/very small tranisitions hidden in the transition track. Transitions other than cross dissolve work fine.

This is crazy. I'm a new Premiere user (6.0) and I use Cross Dissolve all day long. Are you sure (I know you said you were) that the clips are overlapping evenly. I usually let them overlap about 2-3 frames to have a slow, smooth transition.
