PRI going into Line Loopback Active

I experienced a problem with a 412 on 2.1 (Special Build to counter the VCM channels not releasing), PRI going into "Line Loopback Active". Happened twice durring a six month period, fixed it by manually putting the line into Loopback and then Loopback Off, trunks came right back up both times. Went to Avaya nad they say that the CO is causing this. Thought so, until it happende 3 weeks after I installed another 412, this time with 3 primes and today, same scenario with this one. Any Ideas? I'm affraid that if I return to Avaya, they will say the same thing, but this time I am running sysmon to try to catch the dialogue.
