Phone automatically going into Voice Call Deny

I am running a Modular ICS 4.0 system, and I'm having trouble with one of the phones randomly going in Voice Call Deny mode. It's happened once last week, and twice this week.

Is there anything that could be causing this to occur besides the phone being manually set to Voice Call Deny with Feature 88?

I have no formal training working with this phone system. I've been mostly figuring it out as I go, so any help would be greatly appreciated.

Maybe there's a feature button programmed with F88? Use F*0 to do a button inquiry. Could also be user error in dialing.
