What counters are most commonly used when configuring Performance Monitor?
 
It depends what you are looking for, here is a list of key counters:-

CPU - %Processor Time (Over 80% = Bottleneck)
Processor Queue Length(Sustained above 2 = Bottleneck)
Interrupts/sec (Dramatic increase indicates hardware probs)

RAM - Pages/sec (Not be more than 20)
AVG. Disk sec transfer (Should not be more than 10% If it is then you need to install more RAM)

Disk Activity - Disk Queue Length & AVG. Disk/sec transfer

Increasing values over time indicates that the disks may be becoming overloaded with I/O operations.
You need to switch on disk counters as they are off by default byusing the following command:-

DISKPERF -y (Switches ON)
DISKPERF -n (Switches OFF)

Network Activity - Bytes Received/sec
Frames Received/sec
Frames Rejected/sec

There should not be very many frames rejected.
