Patients dialing 8 to call out long distance is not working

At our hospital we have it so patients on there analog patient phones can use 8 to get an outside operator for long distance. Today it just rings fast busy. Anyone know where to look for this in the switch?
 
If you have attendant consoles, Sometimes they inadvertently press the "Trunk Group Busyout" key which could cause this problem. Other wise you could check the members of the trunk group that "8" is accessing. Perhaps a t-1 is down.
 
A FAST BUSY IS AN INDICATION OF ALL TRUNKS BUSY, I AGREE WITH MRPEDERMAN SOUNDS LIKE T1 IS DOWN OR IF THERE COPPER TRUNKS ALL BUSY FOR SOME REASON
