OWA gets HTTP Error 403 - Forbidden 1

I tried to help someone to fix ActiveSync on exchange 2003. I followed the MS article - Exchange ActiveSync and Outlook Mobile Access errors occur when SSL or forms-based authentication is required for Exchange Server 2003. The ActivieSync is working now, but the OWA doesn't (before owa worked).

When trying to logon OWA, we received "You are not authorized to view this page. HTTP Error 403 - Forbidden". I have a document how to setup OWA, OMA and activiesync as below. I have searched Google and tried many suggestions, but can't fix it. Any suggestion?
