Outlook forms guru anyone ??

Were trying to create a public form where folks can Request Classes, Training, Seminars in the format below, does anyone have a basic one that's similar or how we can modify an existing one to do ?? This would start from
the requestor then get routed to the next Dept head, then final Dept head.

To modify an existing form, you want to start by opening the form and then choosing:
Tools > Forms > Design this Form
