Outlook forgets my login credentials

I am making an assumption that this is the Corporate Workgroup installation of Outlook. If so check under TOOLS - SERVICES - MICROSOFT EXCHANGE SERVER
Select PROPERTIES and then ADVANCED
See if the Logon Network Security is set to NT Password Authentication. That way you are validated by your NT logon credentials when you logged on to the system.
If you purposely have it set to NONE then all three fields will remain blank and there is no way to populate those fields that I am aware of. joegz
