Outlook - Creating Forms

I wonder if anyone can help. I am trying to create a form that is used to capture information. The form is sent to another user for them to complete and then return to a specific mailbox.

I have managed to create a form already but when details are entered into the text boxes, the data is not held.

Anyone any ideas?
