Outlook 2K SP3 empty deleted item did not remove from server

I'm assuming that you are referring to the Deleted Item retention setting on the Private Information Store properties.
I think the lowest you can set this is 1 day...maybe 0. It will still keep the mail in there until it has a chance to run the online defragger (ese99). You should see an event log note when it runs. There is a way to change the frequency of this process by changing a registry key. I'll see if I can find that MS KB article for you.
