Hi,
Is it possible to use an Outlook 2000 form to send a standard e-mail to various recipients which has a list of questions for them to answer - some being checkboxes for Excellent, Good, Poor, Very Poor in answer to some questions, others being text boxes for users to put in freeform comments.
The e-mail needs to be easy for the recipient to complete and return it to the sender's e-mail address.
