The out-of-office assistant does not work outside of the LAN for MS Outlook. It works when someone within the LAN send an email but not for those sending emails from the outside.
The out-of-office assistant usually send a message to the sender of an email if the receipient is not in and the receipient choose this option.
 
I take it you are using Exchange as the server.
Exchange has a feature to prevent 'Out Of Office'messages to be propagated to the internet
 
Under Global Settings go to Internet Message Formats then in the right colum right click Default, go to Properties, then the Advanced tab check off "allow out of office responses", "auto replies" & "auto fowards".
