OFT File Outbox problem

We have Exchange 2003 and Outlook 2003. We have created a custom form (.oft file) and it works great. The only issue we have is when you use the form it sits in the Outbox. Outlook never sends it. It never gets to the recipient. There are no pst files involved and I can duplate on any computer in the office. I have taken that same form to a different location that uses Outlook 2007 and Exchange 2003 and it works fine.

Does anyone know why a form or OFT file would not be delivered properly? Is there something I have to turn on in the Exchange server to allow custom forms to be sent?

Is the form published to a forms library at all?

We just opened Outlook, created an email the way we liked and saved as an oft file. I don't think we actually published it in any certain file.
 
There you have it. Where is the recipient supposed to get the form from? You don't want the form being sent every time, as that makes for much larger emails. Look into publishing the form to the Organizational Forms Library.

We created a button in Outlook and assigned the form to it. It was easier than going to Tools, Forms and hunting it down.
I was out two days but I'll look into your suggestion.
