New contact showing "no object found" with custom form

Ok I have created a custom form and assigned it to our cust contacts database. When users click on the form the custom form pops up no problem. However, if a user clicks on New then new contact we get the message "The object could not be found" everytime. We cannot sus this one so if anybody has any ideas I would be very grateful.

Just to make sure we understand things correctly:
- You have a custom form tied to a Public Folder
- When the user goes to the PF and clicks FILE>NEW>CHOOSE FORM, you get the error?

When you published the form, was it published to the Organizational Forms Library, the PF itself, or both?

Also, when you update the form, it's important to update the version number on the form or clients won't download the updated version.

Form published to the org form library and we are using revision numbers by ending the form name with a 1,2, etc etc..
 
Okay, I would then verify that a previous version has not been published to the PF itself.

That's about the only think I can think of, but that would normally just cause the older version to open - not give an error.

Your thinking along the same lines as I am so I guess I will have to go over published form names again.
