.NET 4 VB how can I scan a document like a form and read the fields

this form is not filled in yet I want to make a program that can read a blank form and maybe create and electronic version of it.
Like a patient form at a doctos office
so it would have Name, address etc
then if it had check boxes be able to figure out th words like BP and Pulse

is that possible?

Yes it is possible, however you would need an OCR (Optical Character Recognition) library for .NET. I've just had a quick look on google and there are a number available. You would also need a TWAIN driver / library to handle the scanning.
 
I've just had a quick look at a few on ComponentSource - they are not cheap!!!
