NEC DTERM IP - R.I.P

I have a phone that is working properly except is shows "R.I.P" on the display.

Any suggestions on how to remove it? I've deleted the phone and re-built it and the error hasn't gone away.

We removed the extension in the system but forgot to remove it from the ACD side.
