Multiple Fields on a webpage to one field in Database

I don't is it possible or not. Can I get values in different fields and combine it and store in a database in one field?

For example, First Name :- Ashish
Last Name :- P
Middle Initial :- Smith

This would be asked on the web page and get stored as

Full Name :- Ashish P Smith

in the Database.
How can I do this? I know it has to be possible but am not so sure how to do it.
