I've noticed recently that I have been getting a lot of missed packets in the system monitor of various sizes ranging from 23 to 138 packets lost 5 or 10 time a day is this something I should be worried about I'm using a IPO406 firmware V3.1.29 with a DS16 firmware 5.1.29 and a ip400 8port analog firmware 5.1.2901

This means the IP Office was too busy with other tasks as sending packets to the sysmonitor tool.
Nothing to be worried about but you could narrow the filter in order to get less information in sysmonitor from IP Office.

23 to 138 out of how many - I would suspect several thousand. Missed packets pretty normal on any Ethernet network, its the proportion that counts. For VoIP traffic under 2% is the target.
