MAPD/TSAPI Error Question

I am using a Definity G3si R9.02 with a MAPD v11 DLG and after a random period I start getting Universal Failure events for some switch extension informing me that the maximum number of outstanding requests has been exceeded; I gather what is happening, but I can't figure out why and/or how to fix it. I've checked the load of mesages to and from the definity and it's minimal only about 10k per 30 min period.

Ahy ideas?

This is a message which appears when have been a link down and has not yet recived a confirmation event.The error code in the CSTA Universal Failure event is set to Resource_out_of_service.
