from past experience - and I could be wrong on the solution here - when you have the concurrency remote set too high, qmail sends mail faster than it acknowledges reciept of the message causing duplicates.
I know there was an ongoing battle between qmail admins and DJ Bernstein over this, and I don't know how it turned out.
