Look up Values changing

I have had this occur only a few times but it is driving me to drink (short trip) since I cannot replicate the problem or figure out what is causing it.

Situation: Combo box used to lookup values from a table. Table contains a Primary Key, a description field, an Active (yes/no) field and, occasionally, other pertinent fields. The Primary Key is the bound column (not displayed) and the description field is displayed. The combo is set to 'limit to list'.

Problem: The description value is deleted or changed in the lookup table although the rest of the record and it's primary key are unaffected.

When this happens, I simply check the documentation and 'fix' the incorrect or missing value by reentering it.

There is always the possibility that the user could be getting into the back end, but I have pretty much ruled this out by vigorous interrogation (rubber hoses, drugs, etc).

Any idea how this is happening?

Could it be that your form is bound to a table and that you're not navigating out of the current record? So any changes you make are made on that record?

All the controls on the input form are unbound and not written to the table until all edit checks have been performed.

Your response did get me thinking however and I believe I have discovered how the changes are happening.

The process in question tracks Infection related information for our residents (residential treatment center for children). The data entry form also contains a subform showing historical information on previously recorded infections. The System, Location, Cause and Diagnosis information (literal values) are pulled from the lookup tables.

Upon review, I discovered that I had not locked the subform to prevent the user from making changes; and, obviously, someone thought they could edit the previous records. Doing so changed the lookup values. I'll chalk this one up to a 'senior moment' and make sure I don't repeat the mistake.

Obviously, I'll be spending some time today reviewing other data entry forms to make sure I have locked the subforms.
