latency issues

Hi there I have exchange 2000 running under Small business server. The issue is that when sending emails out to other people over the internet and intranet it takes from 5mins to an hour for the recipient to recieve the emails. This issue comes and goes. I am wondering if it could be the ISP or some setting that I have not configured properly. Can anyone help. Thanks.
